(AM) Fundamentals of Dental Recordkeeping

Presented by: Dr. Rana Badewy and Dr. Parvaneh Bahrami (RCDSO)

Description:
Professional, legal and ethical responsibilities require that dentists maintain complete records documenting all aspects of the patient’s dental care. The chart “tells the story”, beginning with the patient’s medical and dental history, continuing through diagnostic procedures and diagnosis, consent to treatment and treatment plan, and on to progress notes and referral documents. Charting is, quite simply, part of care. The RCDSO’s Dental Recordkeeping courses offer practical advice for understanding the dental recordkeeping requirements in Ontario and how to incorporate them into daily practice.

Learning Objectives:
Upon successful completion of this course, participants will be able to:

  1. Explain the six components of a patient record that must be included to satisfy professional, legal and ethical expectations for documentation.
  2. Describe the content that must be included in each of the areas of the patient's record.
  3. Identify opportunities to strengthen their own practice and apply the fundamental principles of dental recordkeeping required to demonstrate they are practicing to the standard of care.

Important - Knowledge Assessment: Minimum score required
Participants must successfully complete a multiple-choice knowledge assessment after attending the session.

  1. Participants have two weeks after the session to begin their assessment(s), and once they begin, up to two hours to complete the 25 questions. A minimum score of 18/25 is required.
  2. There is one opportunity to re-take the knowledge assessment for those who score lower than 18/25.
  3. Certificates are only provided for those who achieve the minimum score. In other words, if a dentist attends the course but does not attempt or does not successfully complete the knowledge assessment, they are not entitled to any CE points.

Attendees who do not attempt the knowledge assessment or who do not successfully complete the knowledge assessment do not receive any certificate for their participation.

Additionally, you will also learn the steps and wording required to help encourage mental wellness in addition to self-care strategies. To understand why we all behave as we do, we need to learn about the neuroscience behind our actions so we can interact with everyone in our lives, from patients to team members to friends and family, with awareness, mindfulness and just a little more grace.

Learning Objectives:

  2. Identify details that may be overlooked and may lead to miscommunication and conflict.
